SOME QUESTIONS OF PATHOGENESIS OF CHRONIC VENOUS INSUFFICIENCY DURING POST-THROMBOTIC DISEASE

In the article date of bibliography in late years on problem of pathogenesis of chronic venous insufficiency (CVI) during post-thrombotic disease (PTD) were analyzed. Pathogenesis of PTD is very intricate. CVI during the PTD progresses continuously, and, on opinion of many authors, basic causes of initiation of CVI are absolute valvular insufficiency of deep veins, alteration of venous outflow because chronic occlusion or local constriction of deep veins which remain after the best recanalization. PTD is characteryzed with severe CVI and often is complicated with trophic ulcer. In late ten-years some fundamental investigations were made, these allowed to studied early unknown molecular and cellular mechanisms of pathogenesis of venous trophic ulcers. There is given hypothetical scheme of initiation of trophic ulcer: phlebo-hypertension - leucocytes aggression - eluding of cytokines and other active matters - chronic inflammation - injury of tissues. The study of lymphatic system during CVI showed deep damage of drenage function of it.

During complex therapy under CVI must be consideration of pathogenic ways of initiation of CVI and complication - venous trophic ulcer.
