Extensive liver resection in locally advanced cancer

In this paper we analyzed the experience of extensive liver resections in 31 patients with locally advanced liver cancer complicated by rupture of the tumor with peritonitis or internal bleeding (17 patients) and the threat of tumor rupture (14 patients). Patients diagnosed with liver tumor rupture operated in urgent procedure, and patients with liver cancer risk rupture - in the next 1-3 days after confirm the diagnosis and, if necessary, preoperative preparation in ICU.

Five patients died during surgery due to massive intraoperative blood loss, and hemorrhagic shock and DIC syndrome. After operations in the immediate postoperative period, 4 patients died as a result of the decompensated acute liver failure and multiple organ failure. Postoperative mortality was 29% (9 of 31 patients).

Extensive liver resections in life-threatening complications of locally advanced cancer of the liver in emergency or urgently rescues from certain death more than 70% of operated patients on and get 2-year survival rate at least in 50% of cases.
