CLINICAL CASE OF DIABETES MELLITUS IN A CHILD 14 YEARS AFTER KIDNEY TRANSPLANTATION

Post-transplant diabetes mellitus (PTDM) is a serious metabolic complication that develops in recipients after kidney transplantation. Its pathogenesis i multifactorial, combining immunosuppressant-induced insulin resistance with impaired insulin secretion by pancreatic beta cells. The development of PTDM is associated with a significant reduction in patient and graft survival and an increased risk of cardiovascular disease and infectious complications, which accounts for the close attention paid to this problem in transplantology. The incidence of PTDM varies widely, from 5% to 40%, and depends on several factors. These include recipient baseline characteristics (age, overweight, heredity), the immunosuppressive regimen used (particularly high doses of glucocorticoids and tacrolimus), and the duration of follow-up. The peak incidence of diagnosis occurs during the first year after surgery, when immunosuppressive therapy is most intensive. The article provides a detailed analysis of current data on PTDM prevalence, clinical features, and treatment approaches. Particular emphasis is placed on the challenges of diabetes management in transplant recipients, which require maintaining a delicate balance between antidiabetic and immunosuppressive therapy. The authors support the theoretical discussion with their own clinical observation of PTDM in a 14-year-old adolescent.
